How to create an on-call schedule: manual, spreadsheet, and app methods

· OnCall Generator

Creating the on-call schedule is often invisible work concentrated on one person in a hospital or department. You must collect everyone’s requests, avoid conferences and outside assignments, prevent imbalances in frequency and burden, and handle swaps after publication. Doing all of that by hand every month is difficult. This guide compares three approaches—manual planning, spreadsheets, and a dedicated app—and explains principles that make any method fairer.

Compare three ways to create an on-call schedule

MethodBest suited toMain challenges
Manual (paper or whiteboard)Teams of about three or four people with few on-call slotsRequests are easily missed, the basis for fairness is not recorded, and rebuilding the schedule takes substantial work.
Excel or another spreadsheetTeams that need a particular table format or already have spreadsheet workflowsRequests still arrive elsewhere, such as paper or chat; assignment remains manual; formulas and macros can depend on one person.
On-call scheduling appTeams that want to automate both availability collection and fair assignmentMembers need an introduction to the workflow, although proxy entry means not everyone has to install the app.

A spreadsheet template can make the final table easier to format, but it does not solve the two most time-consuming steps: collecting requests and assigning shifts fairly. A purpose-built app can automate both.

Three common problems with manual and spreadsheet scheduling

  • Requests are scattered. — Transcribing requests received on paper, in person, by LINE or another chat app, and by email creates omissions and copying errors.
  • You cannot explain “Why is it always me?” — Even when counts are equal, Friday or pre-holiday shifts with greater burden or higher pay can be concentrated on one person. Manual planning rarely tracks the resulting pay difference.
  • Post-publication swaps are not reflected. — A swap agreed verbally may never reach the official roster, causing confusion on the day.

Five principles for a fair schedule, whatever method you use

  1. Fix the request deadline. — Set a consistent date each month, such as a particular day of the previous month, and handle later changes through the swap process.
  2. Write the rules down. — Decide weekly limits, monthly day- and night-shift limits, and minimum spacing between duties—for example, two clear days—in advance.
  3. Balance rates as well as counts. — If Friday, weekend, or holiday-related slots pay more, also control the maximum difference between members’ total on-call pay. For a public holiday, classify the rate from the following day as described below.
  4. Define incompatible pairs. — Set pairs that must not share a slot, such as combinations needed to maintain a safe balance of senior and junior physicians.
  5. Define the swap procedure. — Prepare a proposed swap, ask the other person, and after agreement update the official roster and share it again with everyone.

How holiday day- and night-shift rates are classified

A public holiday is not automatically priced like Sunday. OnCall Generator first checks whether the following calendar day is also a day off:

  • The next day is another public holiday, Saturday, or Sunday: use the Saturday day-shift and night-shift rates (saturdayDay / saturdayNight), because the working conditions match a Saturday followed by a day off.
  • The next day is an ordinary weekday: use the Sunday day-shift and night-shift rates (sundayDay / sundayNight), as before.

For example, May 3 followed by the May 4 holiday uses the Saturday rate. A substitute holiday on Monday followed by an ordinary Tuesday uses the Sunday rate. The same calculation feeds the total-pay display, pay-gap calculation, and automatic optimization.

How app-based automation works: the OnCall Generator example

The free OnCall Generator app turns the principles above into one workflow with three steps.

  1. Create a room. — Choose the month and set weekly and monthly limits, spacing, incompatible pairs, and pay rates.
  2. Collect requests with a PIN. — Members use a phone or browser to select Available, Preferred, Difficult, or Other work. The organizer can enter requests by proxy for anyone who does not use the app.
  3. Finalize in one tap. — The app creates a roster that obeys the rules, maximizes preferred dates, and balances shift counts and pay. The result can be distributed as a PDF.
